*** THIS POSITION MAY BE ELIGIBLE FOR UP TO A $10,000 SIGN-ON BONUS (restrictions may apply) ***The Physical Therapist provides comprehensive professional physical therapy service in accordance with the mission and objectives of Nascentia Health. Although this position primarily serves Jefferson County, visits to surrounding counties may be required as needed.Performance Responsibilities and Standards:1. Ensure quality and safe delivery of home health services.Provide safe Physical Therapy interventions to clients/families in accordance with agency policies and procedures.Provide client assessments using best clinical practice.Develop appropriate plan of care.Maintain client care records using approved documentation and format.Documentation reflects care and interactions.Documentation is completed in a timely manner.OASIS documentation is completed within regulatory timeframes.Documentation is clear & concise.Follow professional code of ethics and standards of practice.Manage a diverse client caseload.Communicate client care issues to interdisciplinary team2. Assume responsibility for supervision of para professionals relevant to patient care planMonitor job performance of home health aides and delivery of home health aide services3. Comply with Nascentia Health policies and procedures.Maintain Nascentia Health productivity standards.Maintain adherence to agency policies and procedures including Corporate Compliance, Ethical Standards, and Confidentiality (HIPAA).Demonstrate knowledge of federal, state and local government rules and regulations.Demonstrate knowledge and use of infection control practices.Adhere to uniform policy.Adhere to quality improvement standards.Keep personnel requirements up to date, i.e. health requirements, professional license requirement, and other Nascentia Health requirements.Demonstrate skills competency through periodic evaluations.4. Participate in personal growth and development.Participate in education in-services and meetings.Maintain and update clinical knowledge on an ongoing basis.5. Present a professional image to clients and the community.6.
