Loyal Source Government Services is seeking a dedicated and skilled Licensed Practical Nurse (LPN) for an opportunity in Auburn, NY.The successful candidate will be responsible for providing covered services to both assigned and unassigned patients presenting for care at the site. This includes evaluating and caring for patients in accordance with licensure, certification, and functional statements, collaborating with PACT staff to develop comprehensive health care and care management plans, and managing clinic workflow to ensure efficient patient movement through the clinic environment.Key Responsibilities:Provide evaluation and care consistent with licensure, certification, and functional statements to patients assigned to PACTs.Collaborate with PACT staff to develop comprehensive health care plans and care management plans for patients assigned to patient panels.Manage clinic workflow, ensuring patients are placed in examination rooms in a timely manner.Provide direction to patients as they move through the clinic environment.Perform a variety of specialized clinical support skills and basic patient care services.Demonstrate knowledge of medical terminology, aseptic technique, infection control, and patient confidentiality policies and procedures.Assist all health care providers in performing patient care services and duties pertaining to the effective and efficient delivery of patient-centered care in all clinical areas.Cross-train at least one Clinical Associate PACT Team member as a backup to perform telehealth technician duties, including completion of scheduling training and telehealth-specific trainings.LPN/LVN duties include calling patients to perform clinical reminders prior to the providers' VVC appointments.Requirements:Current, full, active, and unrestricted license in the state in which the outpatient site of care (i.e., CBOC) is located.Otherwise, VA qualification standards for Licensed Practical Nurse or Vocational Nurse.Benefits:Pay: $27.19 per hour.About Loyal SourceLoyal Source is an Orlando-based workforce solutions provider dedicated to delivering elite services worldwide, with a focus in government healthcare, technical and support services, engineering, and travel healthcare.
