$18.00 - $25.00 per hour (depending on experience)

Clinical Forms Specialist Benefits:

The Clinical Forms Specialist is responsible for filling out all
necessary patient medical forms, accurately and in a timely manner,
while adhering to strict guidelines and regulations to maintain
patient confidentiality.
Clinical Forms Specialist Responsibilities:
Accurately input patient medical history, diagnoses, medications,
treatment plans, and other clinical data requested into the EMR
system.
Review and complete all required clinical forms (i.e.: FMLA, PFL,
disability), ensuring accuracy and completeness.
Regularly review completed forms for consistency, clarity, and
adherence to medical documentation standards, identify and correct
any errors or missing information.
Maintain strict confidentiality of patient medical records by
following HIPAA guidelines and protecting sensitive
information.
Communicate with physicians, nurses, and other healthcare providers
to clarify clinical information and ensure accurate
documentation.
Stay updated on all relevant healthcare regulations and policies
regarding medical documentation and reporting.
Perform other related duties as assigned.
Clinical Forms Specialist Qualifications:
Associates Degree and two years relevant patient services
experience or equivalent combination of education and experience
required in a health care setting.
Knowledge of medical terminology, medical authorization,
familiarity with medical coding and excellent written/oral
communication skills required.
Two years experience using Electronic Medical Records (EMR).
Experience using Medent preferred.
Two years experience completing medical leave/disability related
documentation.
