  The Clinical Forms Specialist is responsible for filling out all necessary patient medical forms, accurately and in a timely manner, while adhering to strict guidelines and regulations to maintain patient confidentiality.
  
  Clinical Forms Specialist Responsibilities:

  • Accurately input patient medical history, diagnoses, medications, treatment plans, and other clinical data requested into the EMR system. - - - -
  • Review and complete all required clinical forms (i.e.: FMLA, PFL, disability), ensuring accuracy and completeness. - - - -
  • Regularly review completed forms for consistency, clarity, and adherence to medical documentation standards, identify and correct any errors or missing information. - - - -
  • Maintain strict confidentiality of patient medical records by following HIPAA guidelines and protecting sensitive information. - - - -
  • Communicate with physicians, nurses, and other healthcare providers to clarify clinical information and ensure accurate documentation. - - - -
  • Stay updated on all relevant healthcare regulations and policies regarding medical documentation and reporting. - - - -
  • Perform other related duties as assigned.Clinical Forms Specialist Qualifications:
    
    
    • - - - -
    • Associates Degree and two years relevant patient services experience or equivalent combination of education and experience required in a health care setting. - - - -
    • Knowledge of medical terminology, medical authorization, familiarity with medical coding and excellent written/oral communication skills required. - - - -
    • Two years experience using Electronic Medical Records (EMR). Experience using Medent preferred. - - - -
