• Certification Details
• ACLS - Advanced Cardiac Life Support, AHA only
• BLS - Basic Life Support, AHA only
• PALS - Pediatric Advanced Life Support, AHA only
• State License 1 Verification - Primary source verification required. State license is not required for CNAs or PCTs
• Color Blind Testing Online - Required within 1 year of contract start date
• Fit Mask - Must be updated annually and completed within 1 year prior to start
• Negative TB or Chest X-Ray - PPD or IGRA test within 12 months of start
• Rubeola (Measles), Mumps, Rubella - Positive Titers or appropriate vaccinations
• Hepatitis B - Positive Titer, Completed Series or Signed Declination Form
• Varicella - Positive Titer, Vaccinations or History of Varicella
• Flu Seasonal Vaccination/Documentation - Vaccination or signed declination form
