Job Description:

Be the one to make a difference for 28 million members as a clinical professional on our Medical Management/Health Services team.

Key Responsibilities:

--- Reside in Lewis, Oneida, Madison, Chenango, Otsego, or Herkimer County, NY

--- Conduct assessments with members, caregivers, or providers to develop care plans

--- Monitor service delivery and follow up with members, caregivers, or providers

--- Authorize and coordinate referrals for services

--- Ensure seamless delivery of provider services and identify plan deficiencies

--- Assist in coordinating informal services to integrate into member care plans

--- Collaborate with discharge planners, physicians, and other parties to ensure discharge plans

--- Assist members with filing and resolving complaints and appeals

--- Direct care to participating network providers

--- Participate in care management committees and special projects


Requirements:

--- 3+ years of care management experience preferred

--- Home health, discharge planning, or long term care experience preferred

--- Valid driver's license

--- NYS RN license required
